Report an NDIS visitable site form
As an NDIS provider or registered NDIS provider you must give the required information within 14 days after the end of each quarter. It’s important to note that penalties apply if reporting timeframes are not met.

How to submit your report
To report your site:

Privacy statement
We collect your personal information and the personal information of others, including names, position titles, contact numbers, email addresses, client names, their dates of birth, their health information and decision-making capacity, the class of supports provided and opinions on safety risks. Some of the information we collect is sensitive information.
The Public Guardian is authorised to collect this information under section 14(3) of the Public Guardian Act 2014. We collect this information to assist in delivering the Public Guardian’s functions in relation to adults with impaired capacity for a matter.
We will not use or disclose personal information for any other purpose unless we are required or authorised to under an Australian law or court or tribunal order to do so.
The personal information given to us may be disclosed or stored outside of Australia. By submitting this form, you are indicating the individuals whose personal information and sensitive information is being collected agree to the disclosure of their personal information outside of Australia.
If an individual chooses to correspond with us using an email address supported by overseas servers, this will also result in their personal information being sent overseas. As an alternative, they can provide us with an email address supported by Australian servers (normally ending with ‘.com.au’ suffix).

If you choose not to give us your personal information, penalties may apply under the Public Guardian Act 2014 and we may not be able to monitor any welfare concerns for clients at a NDIS visitable site.
